SUBJECT:	Revised "Extramural Support Transmittal Form" and "Request for Account Number in Advance of Award," Effective November 2, 1998

Recently, an Ad Hoc Committee comprised of school/college and campus research administrators reviewed the electronic versions of the Extramural Support Transmittal Form (T-form) and the Request for Account Number in Advance of Award (88-1 form). As a reminder, a completed T-form is required for campus review and approval of all extramural applications and awards, and the 88-1 form is used to obtain approval for new fund/account numbers prior to receipt of an award from an extramural sponsor or donor. The Committee recommended several changes to the forms. As a result, revised web applications and new PDF versions of both forms are now available through the Research and Sponsored Program’s (RSP) home page (http://www.rsp.wisc.edu/forms). The fill-in-the-blank PDF version allows the forms to be printed in the standard two-page format without having to adjust printing options. While preprinted copies of the forms are also available from RSP, the use of electronic forms is encouraged.

The following changes have been made to the T-form:

Items are numbered to facilitate review of the form and for referencing instructions.
All date fields now hold four digits to anticipate the year 2000.
Mailing Instructions (Item 2) now includes an option for electronic submission.
Project Administration section (Item 7, web version) no longer displays pull-down boxes for activity codes, enter activity code(s) in the field provided.
Required Clearances (Item 13) includes a question regarding matching funds or cost sharing commitments.
The "View Information" section (web version) now prints an error message when required fields are left blank.
All numbered fields (web version) appear on the printout instead of only those completed.

The Request for Account Number in Advance of Award (88-1 form) has been modified to more closely resemble the T-form. Significant changes include the following:

Items are numbered to facilitate review of the form.
The Primary Spending Edit section (Item 5, web version) no longer displays pull-down boxes for activity codes, enter activity code(s) in the field provided.
Co-Investigator information is no longer required.
A Justification (Item 11) and Agency/Sponsor Contact (Item 12) is now required.
All numbered fields (web version) appear on the printout instead of only those completed.

The new T-form and 88-1 form will be effective November 2, 1998, and replaces all older versions. If you choose to make reproductions of these forms for future use, please be sure to use the most current version.
