Request for Assignment of Account Number in Advance
of Receipt and Acceptance of Official Award Document (Form 88-1) |
The purpose of this notice is to advise you that the form for requesting
the assignment of an account number in advance of the receipt of an award
(Form 88-1) has been redesigned to include additional required information
and to afford on-line accessibility through our Web page. The sections
of the form that have been redesigned are as follows:

Sponsoring Agency line, identification of prime source of funding, if applicable,
must be determined prior to the assignment of a 133 or 144 account number.

Co-Investigator information line has been added.
-
The Does Project Involve section was added to ensure required approvals
are in place, including required cost sharing/matching.
-
Optional section for school/college administration.
The policy related to the use of the Form 88-1 is unchanged:
-
It is required only when requesting a new fund/account number.
-
It should be used:
-
instead of charging costs to another source of funding which will have
to be cost-transferred later and,
-
when there is an absolute need for the project to begin prior to receipt
of an award document.

It may not be used to establish Multiple Donor, UW Foundation or any other
discretionary gift account; exceptions require extensive justification
and approval of the Director of RSP.
- A UW Transmittal Form, all pre-award
clearances and cost sharing associated with the project (if applicable) must
be on file at RSP before an account number can be assigned.
Also changed is the Federal Project Cost-Sharing Documentation sheet (Form
80-2) which has been updated to include required signatures by the Chair/Director
and Dean/Director.
Please discard all previous paper/template versions of these forms.
