SUBJECT: New "Extramural Support Transmittal Form" and Addendum Document Effective July 17, 1995

Since July 1, 1987 the University of Wisconsin-Madison has used the current "Extramural Support Transmittal Form" (T-Form) for the purpose of assuring internal review and approval of all extramurally supported applications and awards. Due to administrative and regulatory changes that have taken place, an Ad Hoc Committee composed of departmental, school/college, and campus research administrators was appointed by Interim Graduate School Dean Charles Read, to revise the T-Form and develop the new "Addendum To Extramural Support Transmittal Form" (Addendum) for monitoring intellectual property and financial disclosure certifications. The Committee consulted both University faculty and other major research institutions prior to making recommendations. As a general rule, the Addendum must be submitted with the T-Form.

Effective July 17, 1995 the revised T-Form and Addendum must be used when routing the following documents: (1) applications for grants and contracts (including revised budgets); (2) gifts, grants, and contracts (including additions) awarded without a previous formal application; or (3) non-monetary agreements such as transfers of materials, technology and proprietary information.

Research and Sponsored Programs (RSP) will distribute a supply of the new forms to all academic departments as well as Deans' and Directors' offices at least two weeks prior to the July 17, 1995 effective date. We also have the forms available in some electronic formats. Additional paper copies may be obtained by calling 608-262-3822. Due to the administrative and historical importance of the T-Form and its Addendum, if you choose to make reproductions of these forms for future use, please do so only from an original distributed by RSP or from an electronic copy.

The most significant changes in the use of the T-Form and Addendum are:

  1. It is a single sheet of paper versus the previous multi-part carbon pack.

The "Invention and Patent Agreement" certification was moved from the reverse side of the T-Form to the Addendum and renamed "Intellectual Property Agreement" because it now applies to more than patents. A new certification to monitor compliance with financial disclosure by Principal Investigators (PIs) on Federal projects, as explained in a memorandum dated April 19, 1995 from Provost Wiley to all Faculty and Academic Staff PIs, was added to the bottom portion of the Addendum. Both certifications must be signed by the PI on all Federal projects. Only the "Intellectual Property Agreement" needs to be signed on Non-Federal projects.

  7. An electronic copy of the T-Form, the Addendum and their instructions are available in some formats.
Because the T-Form is a working document as well as a routing document, we have added or improved information wherever possible. For example, a place to indicate the University proposal number (to be assigned by RSP) has been added to the upper left-hand corner. Once RSP has assigned and entered the proposal number on the T-Form, it will be photocopied and returned to the PI, Chair(s), and Dean(s) to provide them with the assigned proposal number and the date the proposal was sent by RSP to the Sponsor. The customized Transmittal Letter, prepared by RSP and currently used to transmit the proposal, will be discontinued in favor of a standard cover letter (copy attached), except in those instances when a customized letter is necessary to communicate specific issues or required changes.

PROJECT INFORMATION SECTION
Item 5. Principal Investigator: Boxes for appointment type have been added to alert staff that only tenured or tenure track faculty are automatically authorized by the Chancellor to serve as Principal Investigators. Exceptions to this policy are on a project-by-project basis and must be forwarded by the staff member's department chair and dean to the Dean of the Graduate School for approval.

 
Item 7. Secondary Spending Edits: Space for secondary spending edits has been added to the T-Form for the convenience of those Principal Investigators whose projects involve more than one department and/or school/college. PIs are reminded that all department chairs and deans, or their authorized representatives, must sign the T-Form if they have faculty, staff, or other commitments to the project.

 
Item 9. Proposals: We have clarified that the amount requested is the combined total of direct and indirect costs for the entire project period.

 
Item 10. Awards: Use the "Other (specify type)" line to identify non-monetary awards such as material, technology, or proprietary information agreements, or other agreements/documents that need to be routed to RSP for an authorized signature.

 
REQUIRED CLEARANCES SECTION
Questions 11-15 must be answered, and when necessary, the most current copy of the appropriate committee's approval attached to satisfy University, State, Federal and Non-Federal Sponsor requirements. None of the clearances are new. The instructions provide the telephone numbers of the appropriate offices to call for assistance.

 
REQUIRED SIGNATURES SECTION
The revised T-Form does not change the actual signatures required. However, the PI is required to complete, sign and attach UW Form 95-7(A) "Addendum to Extramural Support Transmittal Form". The instructions for the T-Form provide a good explanation of each required signature. If you have signature responsibility, please read and be aware of the requirements associated with those responsibilities.
Because of the new procedures for documenting "Intellectual Property Agreement" and "Investigator Financial Disclosure for Federal Projects", Department Chairs and Deans and Directors must return, to the originator, all applications and awards that do not use the new T-Form and Addendum after the July 17, 1995 effective date. T-Forms and Addendums that are missing information and signatures should also be returned or completed before forwarding the proposal or award to the next level.
