In order to comply with the new NIH policy requiring the UW to have a project certification for each PI on file instead of the PI signing the application form, RSP has added the required certification to the end of page 2 (Addendum) to the T-form. In addition, RSP is incorporating a few minor changes that we think will be an improvement over the current version. Those Colleges/Units that use their own version of the t-form will want to incorporate these changes as well.

The revisions to the form are:

• The fields that RSP fills out have been sectioned off. The Proposal Number field has been changed to PALS # for clarity.
• 2. Submission Instructions: option for FAX submission has been added, as well as a question about what electronic system is being used if applicable (i.e., Grants.gov, IES, FastLane, etc.).
• 3. “Contact” has been changed to “UW Contact and Phone.”
• 4. For clarification, “Number of Copies” has been changed to reflect the total number required by the Agency or Sponsor, not the “Original +” number of copies.
• 6. SSN – the social security number field has been removed, as this information is no longer required. Space has been provided to allow the Dean’s designee to approve “Other Appointment” personnel for PI status.
• 7a. & 7b. - UDDS Spending Edits have been expanded from two fields to four fields to reflect the current number of fields in ESIS.
• 9. – Proposals: “Revision of UW Proposal No.” has been changed to “Revision of PALS No.” Since a PALS number now follows the proposal/account throughout its life, the distinction of “proposal no.” was unclear.
• 9. - Under “New Account,” the t-form now specifies “Non-federal Grant or Contract” and a “Gift” checkbox has been added.
• Required Signatures - “PI Exceptions” approval has been removed from the RSP signature line as this approval now falls to the Deans’ Offices.
• On page 2, “Annual Investigator's Financial Disclosure Form for Federal Projects” has been changed to “Outside Activities Report” since this report has replaced the old forms. This is now a web-based system; therefore, the instructions no longer reflect that the report needs to be filed with the Department Chair or Dean. There are several other small wording changes to aid in clarity.
