From application to termination
- Complete application and obtain approvals at the school/college and institutional RSP offices.
**Please include the total amount of fees, tuition and health insurance in the fellowship budget. (See Form Checklist, Section D). Please note that for minority student awards, NIH guidelines indicate that the applicant must be from ethnic/racial groups that have been determined by the institution to be underrepresented in biomedical and behavioral research. Please refer to the Advanced Opportunity Fellowships Eligibility criteria for the list of ethnic and racial groups that are underrepresented in the UW's student population.
- Notice of Award is sent electronically to RSP. Fellow may simultaneously receive an award package.
- RSP will notify the student when the award has been received. Student will be asked to complete an activation notice and payback agreement, if applicable.
**Please note that all applicable human subjects, animal care or biological safety protocols must be approved before the award can be activated.
- Account number is assigned and department notified by RSP.
**Please note that effective September 2005, new account numbers will not be assigned for each year of the project.
- Department payrolls student as a fellow and if necessary, gives student benefits information.
**On the IADS form, continuity is 02 for terminal, Activity is 9, and class code is 5712.
- Department sends third party deferral letter to Bursar's Office each semester. Once tuition and fees bill is received, department double checks amount and charges tuition and fees to the fellowship account.
- If fellowship is to be continued, student completes annual progress report and sends it to RSP for appropriate signature and submission to NIH.
- RSP receives continuation notice of award from NIH. At this time, no additional paperwork is required, i.e. activation notice or payback agreement, with the exception of updated protocol approvals.
- The additional time and dollars will be added to the existing account .
- Department continues to payroll student.
The following steps should be followed after completion of the fellowship
- Department ends appointment through payroll.
- Department completes and student signs NIH termination notice.
- NIH termination notice is sent to RSP for signature and on to NIH.
Note: In most cases, the full institutional allowance and other fellowship expenses categories will have been spent on tuition, fees and health insurance costs. Should a balance remain, it must be expended by the department for the benefit of the fellow within 90 days. See Graduate School shortfall memorandum for more information.
- RSP completes tuition overdraft report and sends it to the Department for review. If it is in agreement with Departmental records, the Principal Investigator signs the form and forwards it to Petra Schroeder in the Graduate School. (321 Bascom Hall).
